What is Disability Insurance?

Disability insurance replaces a portion of your income if injury or illness prevents you from working, providing financial security for you and your dependents.

Plan Rules for Disability Insurance: 

  • Coverage through Mutual of Omaha

  • Group Number: G000CSCP

  • Employee must be actively at work on the effective date

  • Disability insurance will be offset by sick leave, workers compensation, or any other outside income.

  • Long-Term Disability: Evidence of Insurability (EOI) will be required if electing outside of your initial enrollment period.

  • The chart below is a sample of covered services. Please see the Plan Certificate for a detailed listing of services in their entirety.
