What are Wellness Incentives?
An annual reimbursement for covered members who complete one of the eligible screening procedures on your critical illness, hospital indemnity and/or accident insurance plans.
Mutual of Omaha
Mutual of Omaha has a wellness benefit included in the Hospital Indemnity, Accident, and Critical Illness plans. If you or a covered dependent get one of the eligible screenings, you can file a wellness claim. Once approved, you will receive a check for the wellness benefit amount. The wellness incentive can be filed annually as long as your plans are in force.
Guardian
Guardian has a wellness benefit included in the Cancer plan. If you or a covered dependent get one of the eligible screenings, you can file a wellness claim. Once approved, you will receive a check for the wellness benefit amount. The wellness incentive can be filed annually as long as your plans are in force.
